Cancellation Policy:

Your appointment is scheduled specifically for you. Please give a courteous 24 hours notice for cancellations or to change an appointment. If less than a 24hr notice is given or you do not show for an appointment, the session fee will apply in a discretionary fashion and is not billable to insurance.

Current Fee Schedule:

The initial session with evaluation and treatment lasts approximately 75 minutes. Each session thereafter lasts approximately an hour. Payment is required at the time of the visit or insurance billing information given. Checks should be made out to Amy Martin, P.T, and of course cash is gladly accepted. We will be glad to provide you with a receipt for cash payment for tax purposes.

FEES billed to insurance for Physical Therapy are $45 for each 15-minute unit as well as a separate initial evaluation charge. A significant discount is offered at $120 per hour session for cash payment at the time of service. We do not want lack of funds to hinder you from seeking treatment with Holistic Physical Therapy and Energy Medicine. Please ask about special fees for Medicare patients or lower income individuals.

Crystal Bed sessions, if used with no other physical therapy treatment modality, are $60 per session and not billable to insurance.

Amy Martin, P.T. is a preferred provider for many insurances including Regence, Premera, KPS, and First Choice. Amy can also bill Labor and Industries and auto insurances (personal injury protection). A referral from a Medical Doctor or Chiropractor is usually required for insurance billing. We do not accept Medicare or DSHS (Medicaid) insurance.
